I think that they clean so thoroughly beforehand, the dick probably comes out cleaner than it was. I've heard stories of them not eating for an x amount of hours (or maybe just all day), douching, using dildos before the scene, just to make sure it's all clean and ready for action. Literally. I used to watch a scene with a quite muscled and hung Brazilian guy (I assume, because he wore a speedo with the colours of the Brazilian flag), but I lost if and forgot his name.
