I think that they clean so thoroughly beforehand, the dick probably comes out cleaner than it was. I've heard stories of them not eating for an x amount of hours (or maybe just all day), douching, using dildos before the scene, just to make sure it's all clean and ready for action. Literally. It's porn, there's tons of things behind the scenes we don't see.
